To create a Google form.

• Sign in to Google forms with your Google account.
• Change the heading of the form

• Click on the + sign to add questions
• Click on the dropdown menu and pick the type of question field you want
• Once you are through it saves automatically. Click on the send icon and choose the form which you want to send it. e.g embed to a website, send the link and send using e-mail.
